If your last name contains any punctuation, spaces, or suffixes, use them, for example: If your last name is O'Conner, enter: o'conner; If your last name is Smith-Bates, enter: smith-bates; If your last name is Baker Jones, enter: baker jones; If your last name includes a suffix, enter it also, such as Morrison IV, enter: morrison iv or Brown Jr. enter: brown jr.
